Before Surgery

A preoperative visit can help you correctly identify and coordinate the pelvic floor before surgery. You can learn how to contract and fully relax the muscles without holding your breath or overusing your abdominals, buttocks or inner thighs. Starting with the right technique can make the postoperative plan feel much less overwhelming.

After Surgery

Once your catheter has been removed and your surgeon has cleared you to begin, treatment can focus on restoring pelvic floor strength, timing and endurance. Your program progresses gradually from basic muscle control to standing, walking, coughing, lifting and the activities that matter to you.

Why Kegels Alone May Not Be Enough

Pelvic floor recovery is not just about squeezing as hard or as often as possible. The muscles must contract correctly, relax completely, respond at the right time and continue working as you breathe and move. If other muscles take over, the exercise may not train the control you need for real-life activities.

Your program should match your current strength, endurance and symptoms. More is not always better. Excessive exercise or repeated contractions without complete relaxation can lead to fatigue and make it harder to use the pelvic floor effectively throughout the day.

Your first visit is private, unhurried and centered on your comfort. You remain in control of every part of the evaluation.

We Start With a Conversation

Melissa will ask about your surgery, catheter removal, current symptoms, pad use, activity level, medications, goals and any instructions from your surgeon. You only need to share what you feel comfortable discussing.

We Evaluate How Your Body Is Working

The evaluation may include breathing, posture, mobility, hip and core strength, balance and functional movements. A pelvic floor assessment may also be discussed. Any internal rectal assessment is optional, explained beforehand and completed only with your consent. You may decline or stop any part of the assessment at any time.

You Leave With a Clear Plan

You will understand what to practice, how often to practice it and how your plan will progress. Exercises are selected for your current needs rather than copied from a standard routine.

Is urinary leakage normal after prostate surgery?

Urinary leakage is common after radical prostatectomy, especially after the catheter is removed. The amount of leakage and the pace of recovery vary from person to person. A pelvic floor physical therapist can assess your muscle control and develop a program that fits your stage of recovery.

When should I start pelvic floor therapy?

You can learn pelvic floor coordination before surgery. After surgery, exercises typically begin after the urinary catheter has been removed and your surgeon has cleared you to start. Always follow the timing and precautions provided by your surgical team.

Should I do Kegels while my catheter is still in place?

No. Pelvic floor strengthening exercises should not be performed while a urinary catheter is in place. Ask your surgeon when it is appropriate for you to begin or resume exercises.

What if I am already doing Kegels and still leaking?

Technique, timing, endurance and complete relaxation all matter. Some men unknowingly hold their breath, bear down or tighten other muscles instead of using the pelvic floor effectively. An individualized assessment can identify what needs to change and how to progress your exercises into standing, walking, coughing, lifting and other real-life activities.

Is an internal pelvic floor assessment required?

No. An internal rectal assessment may provide useful information about pelvic floor strength and coordination, but it is optional. Melissa will explain the purpose of any recommended assessment and ask for your consent. You may decline or stop at any time.

How many visits will I need?

The number of visits depends on your surgery, symptoms, starting strength, goals and how your body responds to treatment. Your plan will be reviewed and adjusted as you progress.

Does insurance cover post-prostatectomy pelvic floor therapy?

Do I need a referral?
